The sick person with no health insurance shows up at the Emergency room and declares himerself "indigent", gets treated anyway and sticks the hospital with the bill. The hospital goes into the red over all this health care they're giving away and declares bankruptcy, or tries to shut down its Emergency room. The neighbourhood sickens and dies.
