Tylenol doesn't affect me, so I usually take ibuprofen with the Ultram (except last week, when I was taking Naproxen). Usually 800mg ibuprofen, sometimes 1g. It does make a difference.
(Way back when I was getting Vicodin, I pointed out to my doctor that acetaminophen doesn't work for me and asked whether I could get just the hydrocodone and spare my liver/kidneys the trouble of processing the acetaminophen. She said that narcotics are nowhere near as useful in controlling pain by themselves as they are when combined with aspirin, an NSAID, or acetaminophen (pretty much ignoring my "Tylenol doesn't work on me" observation). So I started taking ibuprofen with the Vicodin, and whattayaknow, it started working a whole lot better. So then I asked my dotor if I could get Vicoprofen (hydrocodone+ibuprofen) instead of Vicodin (hydrocodone+acetaminophen) but it turned out my HMO (yes, HMO; this was that long ago) would not cover Vicoprofen and their pharmacies didn't carry it. Anyhow, when I started taking tramadol I guessed that it would behave similarly, so I tried it with and without ibuprofen, and sure enough, it too works better in combination with ibuprofen.)
When we get a chance, I'll help you with the guitar even without getting your help with my roof. *hugs*
