They're not trying hard enough: it should be like this:
1. Gender:
a. Male. [ ] (1 = Strongly disagree, 10 = Strongly agree)
b. Female. [ ] (1 = Strongly disagree, 10 = Strongly agree)
c. Other - please specify. Attach additional pages if required. Use blue or black pen for diagrams.2. Did question 1 disturb you? [...]

Living thigns have a sex (or possibly more than one: and there are some examples of living things that have no sex at all).
Grammatical constructs have a gender (and the various possibilities that apply to living things and sex also apply to grammatical constructs and gender).
I am *not* a grammatical construct.
