As far as the huge medical bill, if the cops find them, sue them. Or, if they turn out to be minors, sue their parents. You might not collect but at least you can ruin their credit rating. Explore whether the court can make restitution part of their sentence. You might also check into what your area offers in the way of services for crime victims.

The mayor's Coordinating Council on Criminal Justice: 410-396-4370. It's more of a referral service, I gather. But since O'Malley signed the transgender-rights ordinance a few years ago, he might be able to light a fire under the police if you can get some contacts from others (see below)the mayor's office # is (410) 396-3835.
The Baltimore state attorney's office victim/witness assistance office: 410-396-1897
Metro Crime Stoppers,410-244-8888; Confidential hotline, 410-276-8888. I suspect it's for things like shootings, but they'd know where to turn for hate crimes.
Gay and Lesbian Community Center of Baltimore, (410) 837-5445 (on the hate-crime aspect of this and also because they'd know who in the mayor's office could help)
Chi Epsilon Sigma, Tri-Ess, a heterosexual cross-dressing support group who may know of resources or key contacts.

If you're a local friend who can accompany him to the police (anniemal?), insist that they ask for witnesses in the immediate area, because someone may be able to identify one or more of the perpetrators or have some details on the cars.
